Chase Health Advance

Chase Health Advance financing provides affordable, flexible financing options. A simple, quick and confidential application process and instant qualifying response time. Chase Health Advance offers: No Interest for 24 months/Extended plan for 48 months with credit lines starting at $5000, but financing for initial treatment starts at $300. No down payment required. Our Provider Merchant ID is available upon request.

OrthoBanc

Orthobanc provides a hassle-free payment to Dr. Crosby. There is no interest charged, and there is not a credit check in order to use their service. It is a service for patients that may choose to spread out their payments across the number of months of their treatment. Payments are electronically transmitted each month from your checking, savings, credit card or FSA savings account. A debit card is also available.

Insurance

We accept most insurance. If your insurance covers orthodontic treatment, you will receive the benefit of reduced personal costs. Many insurance policies have a lifetime orthodontic benefit that is distinct from regular dental insurance. Remember that insurance policies vary, and we will review your insurance to determine the appropriate course of action. Once treatment has started, we will file your claims. To help us with your insurance filling, please complete the insurance questions on our questionnaire.

Please remember that orthodontic insurance is a benefit for you or your child. If, during the course of treatment, your benefits change, the financially responsible party is accountable for all charges.

Medicaid

We also accept Medicaid for the convenience of our patients. Medicaid eligibility for orthodontics is provided for patients 21 and younger and with certain bite irregularities. A free consult is needed to evaluate your eligibility.
